WellSky® Introduces Enhanced Intelligence to the Discharge Process in CarePort Suite of Solutions to Facilitate Superior Care Transitions

These new enhancements equip discharge planners, care managers, and care coordinators with enhanced intelligence at the point of discharge to help them make more informed decisions and ensure patients receive the best possible care and support as they transition from the hospital to home or another care setting.

WellSky and CarePort recently expanded its network with the acquisition of Curaspan. Curaspan clients using CarePort Discharge can now take advantage of an expanded solution set.

Accessing clinical data and encounter history at a patient’s most vulnerable point of transition can help reduce hospital readmissions and connect patients to the most appropriate care.

CarePort Guide, available in CarePort Discharge, gives hospitals and health systems access to a more robust patient choice tool to assist patients and families in selecting high-quality post-acute care. CarePort Guide provides access to proprietary post-acute quality scores, highlights high-performing post-acute networks and collaboratives, and offers more robust agency profiles and guides, all sharable via text and email. By increasing patient satisfaction with the discharge process and reducing discharge delays, CarePort Guide brings a superior post-acute care selection tool to the market and can increase the number of patients attending in-network skilled nursing facilities by 7%.
